S blížícím se obdobím prázdnin a dovolených je vhodné se zmínit o možném riziku střevních infekcí. Frekvence těchto onemocnění zažívacího traktu je stále dosti vysoká a právě v letních měsících obvykle významně vzrůstá.

Nejčastějšími původci těchto onemocnění bývají v našich podmínkách salmonely a shigely. U salmonelóz jsou zdrojem nákazy různá zvířata, vč. ptáků, dále pak nedostatečně tepelně zpracované potraviny. Jde zejména o vejce a výrobky z vajec (majonézy, omáčky, paštiky), zmrzlinu, špatně omyté ovoce a zeleninu, ale i málo tepelně upravené masné výrobky (hot dog, hamburgery).

K prvním příznakům salmonelóz obvykle dochází v rozmezí 6-48 hod od požití. Kromě bolestí břicha, je v popředí především průjem, stolice mohou být vysloveně vodové, u části nemocných může být stav provázen horečkou.V důsledku velkých ztrát vody zažívacím traktem dochází k dehydrataci organismu nemocného dítěte. To se projeví celkovou slabostí, únavou, vzestupem tělesné teploty, ale i ztrátou kožní elascity a oschlými sliznicemi.

V komplikovaných případech může docházet k postižení různých orgánů (mozek, ledviny, srdce, játra) a stav může vyústit v bezprostřední ohrožení života. K uvedeným ztrátám tělesné vody jsou velmi citliví především děti věkového rozmezí 0-3roky.

Základním léčebným opatřením u této infekce je dostatečný přívod tekutin. V domácích podmínkách je podáváme přirozeným způsobem, tj. ústy. Ideální tekutinou je zejména neslazený čaj. Kategoricky se vyhýbáme jiným obvyklým nápojům: mléko, minerálky, nápoje typu coca-cola.

Další opatření představuje dieta. Dáváme přednost starším rohlíkům, sucharům, vhodná může být i neslazená rýžová kaše. V prvních dnech této střevní infekce se absolutně vyhýbáme ovoci a zelenině. I tak vesměs nemocné děti nemají žádnou chuť k jídlu. K dietě a dostatečné nabídce tekutin je možno přidat některý z proti průjmových léků, které lze volně dostat v lékárně (Endiform, Endiaron, ). Velikost dávkování vám určí v lékárně s ohledem na věk dítěte. V nekomplikovaných případech výše uvedená opatření obvykle postačí k úzdravení - během 3-4 dnů pomíjí akutní fáze choroby, upravují se stolice a zlepší se celkový stav nemocného. Dietu udržujeme i v tomto období přísnější, není vhodný rychlý přechod ke zcela normální stravě.

U dětí ve stáří 0-3 roků života je vždy vhodné v průběhu akutního průjmového onemocnění vyhledat lékařskou pomoc. Tyto děti často odmítají pití tekutin a nelze jim je mnohdy podávat jinak než formou infuzí. To však samozřejmě vyžaduje hospitalizaci.

Shigelózy (dysentérie) se v mnohém od salmonelóz neliší. Zdrojem nákazy jsou obvykle nemocní lidé. Onemocnění se manifestuje vesměs po 3 dnech od získání nákazy. Bacily se rychle pomnožují v tenkém střevě, vlastní těžké postižení je však ve střevě tlustém. Onemocnění propuká náhle, s velkými bolestmi břicha, s bolestivým nutkáním na stolici. Ta bývá kašovitá, ale i vodová s hlenem a krví. Není vyjímkou, že počet stolic může být 15-30 za 24hod. Nechutenství, horečka, celková slabost a únava doprovázejí uvedené potíže ze zažívacího traktu. Nemocní jsou opět ohroženi velkými ztrátami vody se všemi důsledky jako je tomu u salmonelóz.

Základní léčebná opatření jsou stejná = dostatečný přívod tekutin a dieta. Je možno opět použít některé protiprůjmové prostředky. V komplikovanějších případech je nutná aplikace antibiotik a dodávka tekutin infúzemi. To jsou však opět opatření, která vyžadují hospitalizaci nemocných dětí.
